Required documents

Requirements for your documents

• Make clear colour scans.

• Scan the entire document and make sure all information is clearly readable.

• Do not upload apostilles or translations of apostilles.

• Give each document a clear file name that describes the document, for example diploma, translation of transcript or passport.

Personal documents

• Proof of identity
  Upload your passport, identity card or residence permit. The document must be valid for at least another 3 months.

• Is the name on your proof of identity different from the name on your diploma?
  Upload proof of your name change, for example a marriage certificate.

• Are you applying on behalf of someone else?
  Upload a completed consent form.

Are you applying for a free credential evaluation for Inburgering, naturalisation or a permanent residence permit? Different requirements apply to your personal documents.

Do you have to complete Inburgering?

• Residence permit
  Upload the front and back of your residence permit. The document must be valid for at least another 3 months.

• Kennisgevingsbrief DUO
  Upload the DUO letter with one of the following titles: Kennisgeving Inburgeringsplicht, Kennisgeving Inburgeringsplicht Wet 2021, Inburgeringsplicht Wet 2021 or Overzicht Inburgeringsgegevens Wet 2021.

Are you applying for a credential evaluation for naturalisation or a permanent residence permit?

• Upload an official document showing your citizen service number (BSN). For example, your driving licence, health insurance card, a BSN confirmation letter from your municipality or an extract from the municipal register.

Education documents

• Diploma
  Upload the diploma you want to have evaluated.
  
  Have you not received your diploma yet? Upload an official document from your educational institution showing that you have obtained the diploma and that the diploma has been requested.
  

• Transcript or diploma supplement
  Upload the transcript(s) that belong to your diploma or the complete diploma supplement.
  

• Translation
  Were your diploma, transcript or diploma supplement not issued in Dutch, English, German, French or Afrikaans? Upload a certified translation of these documents as well.
  
  You can have the translation made by a sworn translator in the Netherlands or another country. You can find sworn translators in the Netherlands in the Register of Sworn Interpreters and Translators.
  
  Did your educational institution also provide an English version of your documents? You do not need a certified translation. Upload the documents in the original language and the English version.

Are you applying for a free credential evaluation for Inburgering, naturalisation or a permanent residence permit? You do not need to upload a certified translation. If you already have a certified translation, you can upload it.

Can you no longer obtain a required education document? Upload a statement from your educational institution confirming that the document is no longer available. The statement must be in your name.

• Are you applying for a credential evaluation of a master’s degree?
  Upload your bachelor’s degree certificate and the related transcript(s) as well.
  

• Are you applying for a credential evaluation of a PhD?
  Upload your bachelor’s and master’s degree certificates and the related transcript(s) as well.
  

• Did you receive exemptions during your education programme?
  Upload the diploma and transcript(s) from the previous education programme on the basis of which you received these exemptions.
